Bulk Rate Pricing Application

Fill out the form below for approval for products at bulk rate pricing! To be approved, you must meet the following criteria: 

1. Have up to date Federal and State Tax forms 

2. Have a LuckyBird Blanks Customer Account (To create an account, click the person icon in the top right corner and select "create an account") 

3. Required to spend at least $1,000.00 a month on LuckyBird products monthly

In addition to filling out this form with your name, email address, and phone number,  please email a scanned copy of your Federal and State Tax forms to info@luckybirdblanks.com. Once received and reviewed, you will receive a confirmation email of acceptance by LuckyBird Blanks. Thank you for being a valued LuckyBird Blanks customer!
